How it works
In plain terms.
Tadalafil is a long-acting PDE5 inhibitor. It works the same way as sildenafil but stays active for up to 36 hours, which is why it can be taken either as needed or as a small daily dose that keeps a steady level in the body.
How to take it
- As needed: one tablet at least 30 minutes before sex, no more than once a day.
- Daily: one low-dose tablet at the same time every day, whether or not you plan to have sex.
- Food does not affect it much. Alcohol in quantity can worsen dizziness.
- Your prescriber picks the dose and schedule.
What to expect
- Starts working in about 30 minutes; peak effect around 2 hours.
- One as-needed dose covers up to 36 hours.
- Daily dosing reaches a steady effect after about 5 days.
Not for
- Anyone taking nitrates or riociguat.
- Men whose doctor has advised against sexual activity because of heart problems.
- Anyone under 18, and women.

Sildenafil and tadalafil (including Kormiia™)
Sildenafil and tadalafil are PDE5 inhibitors. Kormiia™ is a compounded combination of the two prepared by our licensed compounding pharmacy; compounded drugs are not FDA approved. FDA-approved generic sildenafil and tadalafil are also available by prescription.
- Do not take with nitrates (nitroglycerin, isosorbide, or recreational "poppers") or with riociguat. The combination can cause a sudden, unsafe drop in blood pressure.
- Tell your prescriber about alpha-blockers, other blood-pressure medications, and any heart condition. Sexual activity puts extra strain on the heart; ask whether it is safe for you.
- Seek emergency care for an erection lasting more than 4 hours, or for sudden loss of vision or hearing.
- Common side effects: headache, flushing, upset stomach, nasal congestion, back or muscle pain (tadalafil), temporary blue-tinted vision (sildenafil).
- Not for use by women or anyone under 18. Do not use with other ED medications.

As needed or daily, which is better?
Neither is better; they suit different lives. As-needed is fewer tablets. Daily removes the timing question and also treats urinary symptoms of an enlarged prostate. Your prescriber will ask how you live.
Can I switch from sildenafil?
Yes, on your prescriber's instruction. Do not take both.
Does it help with premature ejaculation?
It is not approved for that. Talk to your prescriber about what you are experiencing.
