Increase blood flow
Both are PDE5 inhibitors. They relax the blood vessels in the penis so that, with sexual stimulation, more blood flows in and an erection is easier to get and keep.
Two timelines
Sildenafil works for about 4–6 hours and is taken as needed. Tadalafil lasts up to 36 hours and can be taken as needed or in a low daily dose.

Sildenafil and tadalafil (including Kormiia™)
Sildenafil and tadalafil are PDE5 inhibitors. Kormiia™ is a compounded combination of the two prepared by our licensed compounding pharmacy; compounded drugs are not FDA approved. FDA-approved generic sildenafil and tadalafil are also available by prescription.
- Do not take with nitrates (nitroglycerin, isosorbide, or recreational "poppers") or with riociguat. The combination can cause a sudden, unsafe drop in blood pressure.
- Tell your prescriber about alpha-blockers, other blood-pressure medications, and any heart condition. Sexual activity puts extra strain on the heart; ask whether it is safe for you.
- Seek emergency care for an erection lasting more than 4 hours, or for sudden loss of vision or hearing.
- Common side effects: headache, flushing, upset stomach, nasal congestion, back or muscle pain (tadalafil), temporary blue-tinted vision (sildenafil).
- Not for use by women or anyone under 18. Do not use with other ED medications.

Is Kormiia™ FDA approved?
No. It is a compounded preparation made by our licensed pharmacy for an individual prescription. Compounded drugs are not reviewed by the FDA for safety, effectiveness, or quality. The FDA-approved generics — sildenafil and tadalafil — are also available if your prescriber prefers them.
Can I take these with my blood-pressure medication?
Sometimes, but your prescriber has to know. Never with nitrates or riociguat. Alpha-blockers and some other blood-pressure medicines need dose timing or a lower dose.
How fast does it work?
Sildenafil in about 30–60 minutes for 4–6 hours. Tadalafil in about 30 minutes, lasting up to 36 hours. Sexual stimulation is still required.
